DVA Community Nursing
Professional nursing support may be delivered in the home for eligible DVA clients, following an appropriate referral, assessment and approval.
What community nursing means
Community nursing brings appropriate clinical support into the client’s own home. Depending on individual circumstances, this may involve nursing assessment, wound care, medication support, monitoring or help following a hospital stay.
The support provided should reflect assessed needs, current clinical advice and approved arrangements. It does not replace emergency or hospital care when that is required.
